Ohio Medicaid Information

Eligibility
Medicaid pays for medical care and services for low-income individuals who qualify under the law. There are specific eligibility requirements with regard to income and assets that must be met before one can receive the benefits of Medicaid. Certain exemptions apply to married couples when one of the spouses remains at home. For more information about Medicaid eligibility or to file an application in Franklin County, please contact:

Required Information
The interview process requires approximately 1 - 2 hours. The following verification items are required to process the application:
  • Age and Citizenship Verification: Birth or baptismal certificate, military discharge papers, census records, naturalization papers. (submit one)
  • Medicare and Social Security Cards.
  • Monthly Income Verification: Award letters, copies of checks or pay stubs, or bank statements indicating direct deposit (Social Security, Railroad Retirement, etc.).
  • Private Health Insurance: Copy of card or insurance policy and premium stub. (Blue Cross/Blue Shield, Aetna, AARP, etc.).
  • Bank Accounts and Other Liquid Assets: Past three monthly bank statements for checking or savings accounts; certificates of deposit, credit union, trust funds (including original trust agreement), stocks and broker's statements, bonds (including savings bonds).
  • Life Insurance Policies: Bring the actual policies or request a letter from the insurance company stating the face amount of the policy and the current cash value.
  • Property Tax Statements: Including the home or any other property owned. For any real estate interest bring the deed as well as the property tax statement.
  • Guardianship Papers: Copy of Power of Attorney, or guardianship appointment, plus copy of most recent accounting filed with Probate Court for guardianship.
  • Other Verification needed: Pre-need funeral contracts, burial accounts, and cemetery lot deeds; automobile titles; mobile home titles, etc.
If the client is currently living with a spouse, you must provide the above verifications for both spouses along with current rent or mortgage verification, current utility bills, and proof of homeowners' insurance premium.

Covered Services
Once approved, Medicaid will pay for nursing care, medications, medical supplies, use of necessary medical equipment, furnished semi-private room, meals, housekeeping and laundry. Medicaid does not cover beauty shop services, private duty nursing, private telephone service, cable television services, newspaper delivery or charges for special outings or activities.
